"As per our conversation today, I attach our algorithm, conversion (SOLI) form and payment form. If you meet the requirements as per the algorithm, please take the SOLI form to your issuing state (FRANCE) and request that they fill it out for you, ensuring that it is signed and stamped by a Medical Officer. When this has been done please ensure it is returned to:

We also require a copy of your latest medical application form, we have been advised that for the initial medical examination only, the CEMPN completes the "Formulaire pour demande de certificat d'admission" the JAR MED 160 form. For subsequent medicals the 'Lighter Formulaire pour demande de certificat Medical Visite Revisionnelle form ' is used.
Please note : The UK AMS does not recognise the 'Lighter Formulaire pour demande de certificat Medical Visite Revisionnelle form ' as this is not a JAA MED form and therefore we cannot accept this for a conversion. The UK AMS will require the "Formulaire pour demande de certificat d'admission" form (which is the JAR MED 160 form) to be completed for you latest medical. I have attached an example of this form for your information.
